Memento ut diem sabbati sanctifices.
Num Missam diebus Dominicis vel Festis de praecepto per culpam meam omisi?
Num ad Missam tarde adveni vel ante finem sine sufficiente ratione discessi?
Num voluntarie in Missa distractus fui, vel irreverens in ecclesia?
Num opera servilia diebus Dominicis vel Festis sine necessitate egi vel imperavi?
Num diem Dominicum quiete et sanctificatione colere omisi?
Num Sacram Communionem saltem semel in anno tempore Paschali recipere neglexi?
Domine, non sum dignus ut intres sub tectum meum;
sed tantum dic verbo, et sanabitur anima mea.

Remember thou keep holy the Lord's day.
Have I missed Mass on Sundays or Holy Days of Obligation through my own fault?
Have I arrived late at Mass or left early without sufficient reason?
Have I been willfully distracted during Mass, or irreverent in church?
Have I done or commanded servile work on Sundays or Holy Days without necessity?
Have I failed to rest and sanctify the Lord's Day?
Have I neglected to receive Holy Communion at least once a year during the Easter season?
O Lord, I am not worthy that Thou shouldst enter under my roof;
but only say the word and my soul shall be healed.
